Immunization Policy

Our school follows the state of Arizona mandates for childhood immunizations. You must show proof that your child has received the following immunizations to attend school:

  • DTap/DTP/DT
  • Td
  • Tdap
  • Meningococcal
  • Polio
  • MMR
  • Hepatitis B
  • Varicella

The state of Arizona allows for immunization exemptions for medical reasons, laboratory evidence of immunity, and personal beliefs. If you choose not to immunize your child, you must complete the exemption form, which you can find on the Arizona Department of Health Services website. If your child does not receive vaccinations, please be aware that if a school outbreak occurs, we will not allow your child to attend school until the outbreak is over.
